We need your help! This year our annual end of year Visibility Awards will cover 20 different categories... everything from Favorite Gay TV Couple to Best Coming Out Story to Worst Homophobe of the Year.

The 4 to 5 nominees in each category will be announced in early December, but we don't want to miss any obvious choices. Could you help us identify worthy candidates for the various categories?

Below you'll find a write-in nominee form. Feel free to fill in suggestions for as many (or as few) categories as you want. And you can even use the form again if you want to submit multiple candidates in the same category, or think of someone later.

We'll be taking suggestions via this form through November 26th, so come back and suggest as many nominees as you like! And after you've entered your nominees, feel free to lobby for your favorites through Twitter and Facebook!
